Job Title: Clinical Pharmacist Coordinator Company: Veterans Affairs, Veterans Health Administration Location: WEST HAVEN, CT Description: Clinical Pharmacy coordinator has three major areas of responsibility; clinical program management pharmacoeconomics (formulary) management, and educational program management.  The clinical pharmacy coordinator will develop a vision and goals for the clinical, pharmacoeconomic and educational components of the pharmacy program and will assist with future program development.  Familiarity with VA rules and regulations, VA clinical direction, ethical conduct, and clinical practice guidelines implementation processes will be required in this position. Due to the complexities of the facility, the incumbent will be based primarily at the West Haven campus of VA Connecticut.  Occasional visits to the Newington Campus or other locations of VACT, as deemed appropriate by the chief, Pharmacy Service, will be required.  The incumbent holds a position of high visibility. As such, the clinical pharmacy coordinator must have excellent communication and interpersonal skills. Must have knowledge of and will apply marketing techniques to promote and facilitate adherence to established guidelines. The incumbent will work closely with the chief, Pharmacy Service and Drug Information Clinical Specialist, to perform data analysis to measure guideline performance. As such the position requires computer expertise, particularly in the area of database/spreadsheet programs and data analysis. The incumbent is responsible for providing a highly sophisticated and comprehensive range of clinical pharmacy services to veterans in a highly diverse teaching and research healthcare system. The pharmacy coordinator possesses professional knowledge of drug information, pharmacology, pathophysiology, advanced pharmacotherapeutics, pharmacokinetics, pharmacoeconomics and advanced clinical pharmacy practice in order to provide comprehensive clinical pharmacy services. Job Title: Clinical Pharmacist Pain Clinic Company: Veterans Health Administration Location: WEST HAVEN, CT Description: The Incumbent must be a licensed pharmacist with a Doctor of Pharmacy degree, and completed an American Society Health Systems Pharmacists accredited Clinical Pharmacy Practice Residency, or other equivalent experience. The position requires assistance with Anesthesia Pain Service (direct support to the Pain Clinic), management and maintenance of the Pharmacy Pain Medication Program, participation in the VA Pain Council, and precepting pharmacy students and residents. Medication Dispensing: Acts as a provider within a scope of practice provided by privileges accorded by the Pharmacy Professional Standards Board. Documents patient laboratory results, pertinent subjective and objective information, and formulate assessments and plans in the patient's permanent medical record, Clinical: All patients' interviews and assessment notes are completed appropriately in a manner consistent with the current quality assurance policies. Provides accurate drug information to nurses, physicians, and other health care providers. Possess knowledge of potential side effects of pain medications and their treatment and management strategies. Order and evaluate appropriate laboratory tests including urine and blood toxicology screens. Administrative: Establishes a Pharmacy Pain Medication Program to adequately manage pain for veterans cared for by VA CT providers. The incumbent must effectively communicate all appropriate information and significant events through proper channels and to the Director of the Pain clinic and to the Clinical Coordinator, Pharmacy Services, when appropriate. Receives consults for the Pharmacy Pain Medication Program and schedules an initial visit and appropriate follow up dates. Customer Service: Consistently responds to patients, family members, visitors and coworker requests for questions or assistance in a prompt, timely and cooperative manner.

Preferably with an accredited residency, or equivalent experience demonstrating competency.  In addition to basic dispensing functions and clinical duties, the incumbent will supervise the activities of all pharmacy personnel on duty in order to maintain the most expedient and accurate delivery of pharmaceutical services to patients, providers, and other health care professionals at the West Haven campus of the VA Connecticut Health Care System. The incumbent will share equal responsibilities with the other Night Shift Pharmacy Activity Coordinator by serving as the primary pharmacy responder to disasters requiring activation of the drug cache during the night shift.  Clinical: Evaluates physician’s orders as to appropriateness of therapy, based on medications profiles, lab values and patient history. Provides consultation to physicians, nurses and other health care workers regarding drug therapies in general and specific individual drug therapies. Drug Dispensing: Accurately fills or checks medication dispensing units, prescriptions, intravenous admixtures and hyperalimentation and any other sterile products. Fills or causes to be filled all physician orders and requests in a timely manner. Administration: Continually assesses workload trends on off tours and makes appropriate adjustments in personnel work assignments as required to maintain the most expeditious pharmaceutical services to the Medical Center. Stays in constant contact with pharmacy staff on duty to coordinate all activities of the service line to achieve maximum efficiencies and expedient delivery of pharmaceutical services to the entire Medical Center.  Reports all incidents and information required to maintain continuity of pharmaceutical services between shift changes to the Night shift personnel.
