Job Summary
We are seeking a Clinical Pharmacist to join our dynamic healthcare team. In this role, you will provide comprehensive clinical and pharmacy services, oversee technical staff, and contribute to a collaborative and patient-centered environment. You will work alongside a dedicated team that values compassionate care and teamwork.
Key Responsibilities
Medication Management: Interpret, verify, and accurately transcribe orders into the pharmacy system.
Pharmaceutical Preparation: Compound and dispense medications with precision and compliance.
Therapeutic Oversight: Monitor drug therapy regimens to prevent contraindications, interactions, and dosing errors.
Formulary Compliance: Maintain hospital formulary adherence, optimize therapeutic interchanges, and promote rational drug therapy.
Clinical Consultations: Provide timely and accurate medication-related guidance to healthcare professionals and patients.
Specialized Pharmacy Services: Support clinical programs including pharmacokinetics, anticoagulation monitoring, pain management, renal dosing, IV-to-PO transitions, and nutrition support.
Documentation & Compliance: Ensure complete and accurate documentation of clinical interventions, medication errors, and adverse drug reactions.
Operational Leadership: Supervise pharmacy operations and support the pharmacy team in delivering high-quality service.
Qualifications
Open to new graduate pharmacists with completed hospital internships.
Minimum one year of recent hospital experience or completion of a PGY-1 residency in a hospital setting (acute care hospital experience required within the past year).
B.S. or PharmD from an ACPE-accredited School of Pharmacy required.
Active Florida State Board of Pharmacy license in good standing.
