Job Title: Specialty Practice Pharmacist- Lung Transplant Department: University Hospital | Pharmacy

Scope/Position Summary

The Specialty Practice Pharmacist provides comprehensive pharmaceutical care to pre and post-heart and lung transplant patients primarily in the heart and vascular center through integrated inpatient and ambulatory clinical services, research and teaching. In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, family, pharmacists, nurses, physicians, advanced practice providers, medical students, pharmacy students and other associate hospital personnel. Provides specific education to staff related to transplant pharmacotherapy. Recommends and approves new drug orders, which may include checking/verifying progress notes, informed consent and processing non-formulary requests. Screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food (in cooperation with the Dietetics Department), drug-disease state and drug-laboratory interactions. Participates in appropriate hospital and/or departmental committees relevant to the practice area (e.g., Operations Council, heart and lung transplant patient selection and quality meetings).

Minimum Education Required
Doctorate (Academic)

Required Qualifications
Registered pharmacist in the State of Ohio; board certified; advanced degree (Pharm.D./MS) and residency; and/or equivalent experience preferred (e.g. PGY1 and PGY2 residency).
