Hours: Clinic hours, MondayFriday
Weekend & Holiday: No weekends or holidays
Pay: $124,238.40-$186,347.20 annually, based on years of experience

The Ambulatory Pharmacist plays a vital role in providing patient-centered care in our clinic and community-based settings, managing chronic health conditions and optimizing medication therapy.

What You Will Do

• Partner with clinicians to ensure seamless care of patients across the continuum

• Provide clinical support and follow-up assessments for patients

• Prescribe medications, order labs, authorize refills, and manage pharmacotherapy within established protocols

• Collaborate closely with physicians, nurses, and clinical staff as well as the rest of the pharmacy team to optimize treatment outcomes.

What You Will Need

Required

• Wisconsin Pharmacist License

• Bachelors Degree in Pharmacy; Doctor of Pharmacy (PharmD) preferred.

• One of the following:

  • 3 years of relevant pharmacy experience (specialty, outpatient/retail, inpatient or outpatient hematology/oncology, or ambulatory care), or

  • Completion of an accredited Pharmacy Residency (PGY1).

• Strong communication, collaboration, and clinical decisionmaking skills.

Preferred

• Specialized pharmacist certification (e.g., BCACP, BCPS or similar) or active progress toward certification.
