St. Lukes is seeking an Ambulatory Care Pharmacist to join our team. This position is in the pharmacist-run Medication Management Clinic in Twin Falls, Idaho. This clinic currently manages patients on anticoagulation therapy and GLP -1 medications for weight management and metabolic disease, with additional disease-state management services continuing to expand.
Pharmacists in this role independently manage chronic disease states while working closely with primary care providers.
The St. Lukes Ambulatory Pharmacy Department includes pharmacists embedded in clinics across the health system within the following specialties: internal medicine, family medicine, endocrinology, anticoagulation, heart failure, lipidology, cystic fibrosis, and pediatric specialties.
What you can expect from this role
- An independent clinic-based pharmacist practice with broad scope of practice
- Management of a fast-paced clinic schedule of 12-20 patient appointments per day
- Delivering comprehensive support to providers and patients by answering drug information inquiries, resolving pharmacy-related issues, and providing medication education
- Working as part of a multidisciplinary team with physicians, advanced practice practitioners, dieticians and nurses
- Collaboration with clinical pharmacy technicians
- Opportunities for precepting pharmacy students and residents
- Hours: Monday-Friday 8 am to 5 pm
Preferred Qualifications for this Role
- Previous experience with the clinical management of anticoagulation or GLP -1 therapy preferred
- Completion of a PGY2 Ambulatory Care Residency, completion of a PGY1 Pharmacy Residency with strong ambulatory care focus, or equivalent ambulatory care experience
- PGY1 pharmacy residency graduates with strong ambulatory care training are encouraged to apply
- Current Board Certification in Pharmacotherapy ( BCPS ), Ambulatory Care Pharmacy ( BCACP ), or Geriatric Pharmacy ( BCGP )
