This position offers the chance to significantly impact patient care by managing medication regimens with an emphasis on Population Health and Ambulatory Care, specifically targeting chronic conditions such as diabetes. Responsibilities:

Provide clinical pharmacy services focused on Population Health and Ambulatory Care.

Collaborate with healthcare teams to optimize medication management for patients.

Balance clinical duties with staffing responsibilities, dedicating approximately 60% of time to clinical work and 40% to staffing activities.

Assist in expanding clinical pharmacy programs as the department grows.

Utilize Meditech for patient records and medication management.

Work variable day and afternoon shifts, primarily 8-hour day shifts. Qualifications:

Current Pennsylvania pharmacist license required.

Experience in ambulatory care or inpatient pharmacy preferred.

Completion of residency highly desirable.

Strong passion for patient care and clinical program development.
