The Clinical Pharmacist is responsible for the development, implementation, monitoring and reporting of CMS STARS pharmacy measures, overseeing the central pharmacy refill protocols, and streamlining pharmaceutical patient assistance programs at ArchWell Health. This role with interface with health plans, IT data engineering, and patient care teams. The Clinical Pharmacist will provide leadership and clinical education activities at ArchWell Health that improve value-based care over multiple markets and states in an outpatient senior-focused primary care setting.Essential Duties/Responsibilities
Monitor CMS STARS Medication Adherence measures/Statin measures over several health plansDesign and implement strategies for closing Medication Adherence and Statin gapsHiring and direct oversight of pharmacy technician(s)Develop, maintain, and deploy a standardized onboarding program for pharmacy techniciansMeet regularly with health plan pharmacy representativesDevelop provider education on pharmacy topics as assignedBuild relationships with IT data team and market leaders.Assign and monitor central refill protocol workflow, documentation, and qualityDevelop workflows to assist member entry and re-enrollment into pharmaceutical-sponsored patient assistance programs.Additional responsibilities and duties as requested and deemed necessary by the executive team and VP of Education and Documentation
Required Education and Experience:
Degree from an accredited college of pharmacyA valid, active pharmacy license in state of employment requiredMinimum of 2 years clinical experience.
Knowledge, Skills and Abilities:
Medicare Advantage experience in CMS STARs Pharmacy MeasuresExperience with Electronic Medical RecordsAdvanced knowledge of ICD-10,CPT, and HCPCS codesProficient with Microsoft Office Suite products including Word, Excel, Power Point, and Outlook, and TeamsAbility to travel to visit centers markets at the national level to provide clinical education.Demonstrated strong leadership and team building skillsExperience in clinical process improvementAbility to create, plan, implement and evaluate patient specific care plansExcellent presentation, verbal, and written communication skillsAbility to exercise use of independent judgementSensitivity to the diverse needs of culturally diverse staff, patients, and communitiesExperience in development and maintenance of guidelines and clinical policiesEmbodies and serves as a role model of ArchWell Health's Values:
