About the Role: Job Summary

Serves as a pharmacotherapy expert and provides clinical services in specialized practice area. Functions in the role of consultant and educator of drug therapy to health care team. This position will care for patients in the neonate, child, adolescent, adult, and geriatric age groups. Employee performs clinical duties in accordance with population specific guidelines and adheres to departmental policy and procedures.

Essential Tasks and Responsibilities
  • Identifies and resolves medication related problems in a wide range of complexity through interdisciplinary patient care rounds, profile review and consult management

  • Applies knowledge of medication management and patient assessment skills to intervene and recommend appropriate evidence-based, cost-effective and safe therapy.

  • Collaborate with Medication Safety team to detect, mitigate and prevent medication adverse effects and/or outcomes

  • Supports the hospital's antimicrobial stewardship program by ensuring compliance with formulary restrictions, providing antimicrobial stewardship interventions and educating healthcare providers on appropriate prescribing and guideline adherence.

  • Serves as a subject matter expert in creating and updating evidence-based treatment pathways, guidelines, protocols and order sets

  • Conducts research (IRB approved) and quality improvement projects, including medication utilization evaluations

  • Participates in the pharmacy residency program as a preceptor, preceptor-in training, program director, program coordinator or mentor. Maintains proficiency in preceptor roles.

  • Serve as a pharmacy department resource to liason and collaborate with other departments to improve the medication use system

  • Responds to drug information requests through retrieval, interpretation and communication of pertinent medical literature.

  • Educates patients and families on medication therapy

  • Provides formal and informal education to healthcare professionals, including the GME department, at the local, state and national level to improve prescribing, administration and monitoring of medication therapy.

  • Serves as an appointed preceptor for introductory and advanced pharmacy practice experiences. Prepares students for post-graduate residency training.

  • Develops training experiences for pharmacists in medication consult management, pharmacotherapy and decentralized pharmacy services

  • Represents the pharmacy department during DNV surveys in respective practice area. Collaborates with other departments on reaching compliance of medication management standards.

  • Serves as an active member of the P&T Committee by evaluating medications for addition or deletion from formulary and developing criteria for use and/or therapeutic interchanges. Reviews and interprets the primary literature and effectively presents the pharmacy department proposal(s).

  • Participates in hospital or system-wide committees as the subject matter expert , including but not limited to: Antimicrobial Stewardship Committee, Sepsis, Sedation, Resuscitation, Stroke, or other disease state commitees.

  • Complete other duties, as assigned.

Antimicrobial Stewardship/Infectious Disease

  • Specific responsibilities of Antimicrobial Stewardship/ID Specialist: (1) Collect and report intervention data for Antimicrobial Stewardship Team; (2) Conduct routine pharmacoeconomic assessments of the Antimicrobial Stewardship program across the health system; (3) Coordinate agenda for Antimicrobial Stewardship Committee.
