Job Description
General Purpose of Job:
Responsible for the maintenance, development, and implementation of current and future clinical pharmacy programs and services. Provide clinical pharmacy services, assist in the department's Associate development programs, and perform duties of staff pharmacist.
Essential Duties:
This job description is intended to cover the minimum essential duties assigned on a regular basis. Associates may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
- Perform clinical pharmacokinetic dosing service in a timely and accurate manner.
- Provide Drug Information services.
- Participate in Pharmacy & Therapeutics Committee activities, and other committees such as Quality & Safety, Medication Safety assigned.
- Participate in Antimicrobial Stewardship Committee activites and act as the anitmicrobial stewardship pharmacy leader.
- Assist in the department's associate development programs by presenting and coordinating pharmacist and technician education and training.
- Assist with nursing and physician education on medication issues.
- Write clear objectives for program development with target dates outlined.
- Assist in training, instruction, supervision and evaluation of students assigned to the pharmacy, and participate in department orientation.
- Able to pass IV additive training program, mixes IV solutions and chemotherapy as required using aseptic technique with the laminar flow hood or biological safety cabinet.
- Compounding and supervising the compounding of Non-sterile medications up to and including Moderate Compounding as defined by USP and Sterile medications up to and including Medium Risk as defined by USP .
- Compounding caregivers adhere to the prescription or medical order for the strength, quality and purity of compounding preparations. Compounded products are packaged, labeled and dispensed according to current laws and regulations.
- Perform duties of a staff pharmacist on an assigned schedule.
Job Requirements
General Requirements State of Michigan Pharmacist and Controlled Substance License in good standing Work Experience Clinical pharmacy experience preferred. Experience as a pharmacist preferred. Education Pharm D degree. Graduate of an accredited College of Pharmacy. Specialized Knowledge and Skills Able to demonstrate the knowledge and skill necessary to provide care based on physical and related criteria, appropriate to the age of patients, related to the principle duties and responsibilities of the position. Able to use word processing and spreadsheet software programs. Therapeutic drug monitoring experience preferred.
