M2M Pro, Leading the way for Independent Physicians and Pharmacists to team together. M2M Pro will put Pharmacists onsite in the Primary Care Office to help patients receive Medication Therapy Management to help them better understand what the medications do and how the medication can help in the battle to control Chronic conditions. M2M Pro is a company dedicated to working with ACO’s and Medicare to optimize health services by easily training individual pharmacists to work directly with high risk, contracted patients, their providers and community pharmacists to ensure optimal treatment. In turn, reducing the risk of adverse events, drug interactions and helping the Physicians to meet their MACRA reporting needs. This allows Pharmacists the time to get to know the patients while getting to know the physicians in their community to complete the circle of care. We would love to have those Pharmacists who have a passion for Patient coaching and education come onboard with us.
