Full-Time Home Infusion Pharmacist Nashville, TN

Soliant Health is seeking a qualified and detail-oriented Pharmacist for a full-time interim/travel assignment in the Nashville, TN area. This position will cover a 1316 week maternity leave and offers a great opportunity to gain experience in a growing home infusion environment. Position Overview:

• Setting: Home Infusion Pharmacy
• Duration: 1316 weeks (maternity leave coverage)
• Schedule: MondayFriday, 8:30 AM 5:00 PM; rotates to 9:30 AM 6:00 PM for on-call support
• On-Call: Availability for on-call duties required

Key Responsibilities:

• Review and approve prescriptions for accuracy, completeness, and regulatory compliance
• Perform final checks on filled prescriptions to ensure timely and accurate delivery
• Conduct medication assessments based on patient profiles and lab data
• Support infusion-related therapies by offering drug information and clinical guidance
• Monitor therapeutic drug ranges and respond to internal and external clinical inquiries
• Assist in implementing quality control and cost-containment strategies
• Maintain compliance with HIPAA, federal/state pharmacy laws, and company policies
• Provide leadership support in the absence of the Pharmacist in Charge
• Participate in annual competency verification and continuous professional development

Requirements:

• Graduate of an accredited school of pharmacy
• Active Tennessee Pharmacist license
• Previous experience in a home infusion or specialty pharmacy environment strongly preferred
• Strong clinical acumen, communication skills, and attention to detail
• Proficient with pharmacy software and technology
• Ability to collaborate in a team-driven setting and manage multiple priorities effectively
