Overview
At St. Lukes, our dedicated team of Home Infusion professionals strives to build a positive, supportive, and collaborative culture centered on exceptional patient care and operational excellence. We are seeking a Flex/ PRN Home Infusion Pharmacist to support our growing Ambulatory Infusion Services ( AIS ) and Home Infusion Pharmacy operations in Meridian, Idaho.
This position provides flexible pharmacist coverage for clinical verification, operational support, PTO coverage, and after-hours call support within a highly specialized home infusion environment. The role is ideal for pharmacists who thrive in a fast-paced setting requiring strong clinical judgment, operational awareness, and collaboration across interdisciplinary care teams.
Responsibilities
- Perform clinical review and verification of home infusion medication orders for accuracy, appropriateness, dosing, compatibility, and safety.
- Support pharmacy operations across multiple platforms, including Epic, CareTend, and external vendor systems.
- Assist with coordination and oversight of inventory management activities, including both manual and system-driven inventory processes.
- Collaborate with pharmacy technicians, nurses, providers, and operational teams to ensure timely medication preparation and delivery.
- Support patient care for infusion therapies including antimicrobials, biologics, nutrition support, hydration, immunotherapy, and other specialty infusion medications.
- Provide pharmacist coverage during PTO periods, staffing gaps, and increased operational demand.
- Participate in after-hours and on-call pharmacist coverage rotation as assigned.
- Assist with resolving medication access issues, clinical clarifications, and urgent patient care needs.
- Ensure compliance with applicable USP standards, state and federal regulations, accreditation requirements, and organizational policies.
- Maintain accurate documentation and support quality assurance and patient safety initiatives.
- Contribute to workflow optimization and operational continuity during periods of growth and expansion.
Qualifications
Education
- Doctor of Pharmacy (PharmD) degree or equivalent accredited pharmacy degree.
Experience
- Experience in home infusion, ambulatory infusion, hospital pharmacy, sterile compounding, or specialty pharmacy preferred.
