Saratoga Medical has an exciting opportunity for a Clinical Pharmacist in San Diego, CA . This role provides clinical pharmacy services for patients with chronic pain conditions through medication management, patient counseling, and interdisciplinary collaboration.
Qualifications:
- Doctor of Pharmacy (Pharm.D.).
- Graduate from an ACPE-accredited college/university or possess an FPGEC certificate with active U.S. licensure.
- Current, full, active, and unrestricted Pharmacist license.
- Completion of a clinical pharmacy internship, residency, and/or fellowship accredited by ASHP or ACCP, or experience meeting clinical competency requirements.
- Must meet BLS requirements.
Additional Requirements:
- Candidates must possess one of the following:
- Graduate certificate from an accredited pharmacy residency program within the preceding year; OR
- Minimum 12 months of direct patient care Clinical Pharmacist experience within the preceding 18 months in an inpatient, outpatient, or ambulatory care pharmacy setting; OR
- Clinical privileges as a Clinical Pharmacist within the preceding 36 months in a Military Treatment Facility.
Responsibilities:
- Assess patient response to drug therapy and develop pharmacotherapeutic treatment plans.
- Order and evaluate laboratory tests related to medication therapy.
- Initiate, modify, or discontinue medications in collaboration with medical staff.
- Evaluate medication histories and identify drug-related problems.
- Consult with physicians and other healthcare providers regarding patient treatment needs.
- Document medication-related interventions and adverse drug reactions.
- Participate in multidisciplinary care for patients suffering from pain.
- Perform medication reviews, reconciliation, and polypharmacy management for chronic pain patients.
- Educate patients and caregivers regarding medications and treatment plans.
- Order consults and referrals as appropriate.
- Participate in tele-mentoring, tele-medicine, and tele-consults.
